Job Description:
Home Health RN – Plano/Richardson, TX Pay Rate: $50–$54 per hour
mileage reimbursement at $0.38/mile Schedule: Monday–Friday, 40
hours per week (includes drive time, visits, and charting).
Overtime must be preapproved. Work Location: Field-based –
Richardson, Plano, and Dallas area Contract-to-Hire via A-Line
Staffing Solutions: 13 weeks with potential to get hired directly
after | Full-Time | No on-call or weekends | 28 - 30 units/week Key
Responsibilities: Perform home visits and patient assessments to
determine care needs. Develop and implement patient-specific Plans
of Care (POC). Educate patients and families on health conditions,
medications, home safety, and self-care. Supervise clinical staff
including HHAs and LVNs. Utilize infection control practices to
ensure safety of patients and staff. Maintain accurate and timely
documentation in accordance with agency and regulatory guidelines.
Participate in interdisciplinary team meetings and contribute to
coordinated patient care. Refer patients to appropriate resources
as needed and monitor compliance with payer requirements.
Requirements: Current TX RN license, active and in good standing 2
years of recent Home Health RN experience – required Experience
with OASIS and Homecare Homebase (HCHB) – strongly preferred CPR
Certification – required Reliable transportation, valid driver’s
license, and proof of insurance – required IV care, wound care,
phlebotomy, catheter care – required Graduate of an accredited ADN
or diploma nursing program – required BSN – preferred CHPN
